Complete the Radiant MRI requisition and fax it to (902) 812-0820 or email it to info@radiantmedicalsystems.ca.
The fillable version can be completed on a computer; the print version can be printed and completed by hand.
Who can refer
Referrals are accepted from the following providers, where authorized to order diagnostic imaging:
- Chiropractors
- Nurse practitioners
- Physiotherapists
- Physicians
How to refer
Referrals can be submitted to our team using the contact details below. Please include enough clinical information for our radiologists to confirm that the requested imaging is appropriate.
- Patient name, date of birth, and contact information
- Referring provider name and contact details
- Requested exam and the body region of interest
- Relevant clinical history and the reason for the request
- Any relevant prior imaging or implanted devices
Report delivery
Images are interpreted by Royal College certified radiologists, and reports are sent to the referring provider. Our radiologists are available for consultation to discuss findings and patient care.
Secure access
Images and reports are handled in line with applicable privacy requirements and are made available to referring providers through secure means.
